Approval from the Institutional Animal Care and Use Committee (IACUC) is required for any activity involving vertebrate animals in biomedical research, training, or teaching. Applicants must submit an IACUC application.
A Suspension and Debarment (S&D) form is required for Federal and State funded projects. This includes projects where Mount Sinai has the subaward on a federal or state funded project.
The FCOIR – Research Trigger Form (project-specific form) is used to determine whether any financial interests pose actual or perceived conflicts with a proposed research project. This form is required for every grant and research project/study.
All sponsored project applications and a subset of ISMMS projects are prepared in InfoEd and routed for approval through the department(s) in which there are key personnel each year of the project.
All Mount Sinai principal investigators, co-investigators, mentors, and key personnel must complete an Annual Report of Outside Relationships (also called the Annual Form) disclosing all financial interests that they may have outside of employment at Mount Sinai.
